For a dice room to spawn, two things must happen;
First, the game must decide that a sacrifice room will spawn that floor.
There are six different dice room configurations in the game, each corresponding with a face of a d6 and its own effect.
It can be annoying to remember what each room does, but there are some handy signs that can act as reminders if you have the afterbirth+ expansion.
Dice rooms cost two keys to enter.
Dice rooms randomize many aspects of the game, depending on the number printed on the very large red dice embedded in the floor.
Stepping onto this die activates its effect.
This can only be done once per room.
Perhaps an occasional room layout with an item pedestal, drawing from a new dice room item pool with the various dice in it.
The room could occasionally feature a restock machine.
When the effect of the machine triggers (via explosion or coin donations), the dice pip would be rerolled.
