To log in to mycgs, access the mycgs log in page.
You must be an established provider administrator or provider user in order to log in to mycgs.
If you do not have a mycgs user.
When you log into mycgs, you will find the forms tab.
This option will allow you to access and submit cgs forms through mycgs.
Provider administrators will have access to the tab.
Your supplier number (ptan) was revoked.
If you believe your ptan was revoked in error, contact the national provider enrollment (npe) contractor.
Resubmit the claim after the npe.
